Solace Health Services is authorized to provide HCBS through our 245D designation under Minnesota Statutes §245D.03. We deliver individualized support through DHS-aligned programs for individuals enrolled in Minnesota’s Medicaid waiver programs.
Understanding Minnesota HCBS Waiver Programs
Minnesota offers several Home and Community-Based Services (HCBS) waiver programs to help individuals who require nursing facility level of care remain in their homes and communities. These programs provide services beyond what traditional Medical Assistance covers.
Waiver Programs We Support:
- Community Access for Disability Inclusion (CADI) Waiver: For individuals under age 65 with disabilities who require nursing facility level of care but choose to live in the community
- Developmental Disabilities (DD) Waiver: For persons with developmental disabilities or related conditions requiring ICF/DD level of care
- Brain Injury (BI) Waiver: For individuals with traumatic, acquired, or degenerative brain injury requiring specialized nursing facility or neurobehavioral hospital level of care
- Elderly Waiver (EW): For individuals over age 65 who require nursing facility level of care and choose community living
- Community Alternative Care (CAC) Program: For people over age 65 at risk of nursing home placement (administered through DHS Aging and Adult Services Division)
245D-Approved Basic Support Services:
Under Minnesota Statutes §245D.03, basic support services provide assistance, supervision, and care necessary to ensure health and welfare, including:
24-Hour Home Supports with Skill-Building:
Round-the-clock staffing that includes teaching daily living skills, promoting independence, and ensuring safety
In-Home and Out of Home Respite Care
Temporary care to provide relief for primary caregivers, maintaining the individual's routine and comfort
Night Supervision Services
Overnight awake or sleep-cycle staff to ensure safety and respond to emergencies
Enhanced Homemaker Services
Comprehensive household management including meal preparation, cleaning, laundry, and organization tailored to individual needs
Behavioral & Positive Support Services
Implementation of positive behavior support plans developed by licensed professionals, crisis prevention and intervention, de-escalation techniques
Community Living & Habilitation Programs
Support accessing community resources, developing social connections, building life skills, employment support, and recreational integration
Adult Companion Services
Supervision and socialization to prevent isolation and ensure safety
Individual Community Living Support
One-on-one assistance with community participation, relationship building, and independent living skill development
