Our skilled nursing services are provided by licensed Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who coordinate directly with your primary care provider to ensure seamless transitions from hospital to home. All nursing staff maintain current Minnesota licensure and meet continuing education requirements.
Clinical Skilled Care Services Include:
Wound Care & Post-Surgical Management:
Advanced wound assessment, dressing changes, surgical incision monitoring, ostomy care, and pressure injury prevention
Medication Management & Education:
Medication administration, compliance monitoring, patient/family education, medication reconciliation, and coordination with pharmacies
Catheter Care:
Foley catheter insertion, maintenance, and removal; suprapubic catheter care; urinary tract infection prevention
Pain Management:
Assessment using validated pain scales, medication titration, non-pharmacological interventions, and coordination with pain management specialists
Injection Administration:
Insulin injections, anticoagulant therapy, vitamin B12, biologics, and other prescribed injectable medications
Chronic Disease Management
Glucose monitoring for diabetes, blood pressure monitoring, COPD/respiratory management, congestive heart failure monitoring
Vital Signs Monitoring:
Temperature, pulse, respiration, blood pressure, oxygen saturation, and pain level assessments
Client Assessment & Health Education:
Comprehensive nursing assessments required under §144A.43, development of individualized care plans, disease-specific education, and caregiver training
Long-Term Care Coordination:
Ongoing monitoring of chronic conditions, coordination with physicians and specialists, medication management, and preventive health education
